What if I have a difficult patient population?

It appears that future reimbursement models will be based upon performance. They certainly can't be based solely on performance, but higher reimbursement and performance bonuses are coming into the picture across the country. What if I have a difficult patient population? They just won't listen, won't get their tests done or take their meds as they should. Plenty of counseling, advice, nutritionists, case managers, behavior change specialists helping out and still not getting the results that are expected/needed. Stubborn patients that won't listen or maybe a tougher genetic pool of patients that simply can't be controlled well. Should my effort/process be rewarded or only the results?
