Patients can enroll by completing the currentcare Enrollment and Authorization Form at participating healthcare provider sites.
Encourage your patients to enroll in currentcare by making the Enrollment and Authorization Form available at your office or by accepting forms that have been downloaded and filled out by your patients.
Once your patients have completed, signed, and returned a “currentcare Enrollment and Authorization Form” to the Rhode Island Quality Institute, their health information will begin to be available through currentcare from participating data sources (as of today, most laboratories).
